
Quality Improvement Plan
The Norfolk Hospital Nursing Home has posted its 2026 to 2027 Quality Improvement Plan, which outlines our priorities to improve care, safety, resident experience, and quality of life.
A Quality Improvement Plan, or QIP, is a formal plan that shows how a long-term care home will improve care and services over the year.
It includes clear goals, targets, and actions. These priorities help us track progress, respond to resident and family needs, and continue improving the care we provide.
Our 2026 to 2027 QIP focuses on key areas such as:
- Reducing avoidable emergency department visits
- Strengthening equity, diversity, inclusion, and anti-racism education
- Improving resident experience
- Supporting resident safety
- Reducing falls, pressure injuries, and the use of physical restraints
- Strengthening palliative and end-of-life care
The plan also reflects our ongoing work to provide safe, respectful, and resident-centred care at Norfolk Hospital Nursing Home.

How to read the Quality Improvement Plan
We recommend starting with the QIP Narrative. This gives the clearest overview of the plan and explains the main priorities for the year.
You can then review the Executive Summary for a quick look at the targets, followed by the Progress Report and Workplan for more detailed information.
